Identifies, organizes, and completes performance improvement projects related to medication use. Develops and implements pharmacy initiatives directed at promoting high value, high quality, safe, and cost-efficient medication therapy management in accordance with value-based initiatives.<br><br><b>Job Duties</b><ul><li>Applies subject matter expertise in clinical pharmacy, therapeutic alternatives, utilization management, medication adherence strategies, and Medication Therapy Management to promote positive outcomes in LVHN value-based reimbursement contract programs.</li> <li>Critically reviews drug utilization data provided by insurers and analytic team and assure ongoing performance improvement of the pharmaceutical care services provided.</li> <li>Evaluates patient EMR profiles for appropriateness of medication therapy, conducts patient appointments, and provides and documents patient drug therapy recommendations.</li> <li>Identifies prescribing patterns that may lead to improper drug selection, sub-therapeutic dosage, overdose, patient failure to receive medication, adverse drug reactions, drug interactions, and medication without indication and implement strategies to resolve cost barriers.</li> <li>Interact with patient, providers, and pharmacy to positively impact CMS STARS, PQA, or HEDIS quality measures. </li> <li>Works with Population Health Director and ICC Pharmacy Manager to help implement strategic population health pharmacy plans.</li> <li>Provides education to providers and staff via presentations, newsletters, formal utilization reviews, and one-on-one interactions.</li> <li>Ensures quality of care through formulary and/or medication management and quality improvement projects.</li></ul><br><b>Minimum Qualifications</b><ul><li>Bachelor’s Degree Pharmacy or </li> <li>Doctorate Pharmacy. </li><li>2 years relevant pharmacist experience in patient care or population health or</li> <li>1 year PGY-1 Residency. </li><li>Skilled in critical appraisal of medical literature. </li> <li>Capable of independently reaching therapeutic decisions in patient care and communicating them to physicians and other involved health professionals. </li> <li>Ability to accurately monitor medications. </li> <li>Ability to make independent judgments under stressful situations at times. </li> <li>Identify, organize, and complete performance improvement projects related to medication management. </li> <li>Ability to work effectively in a team environment. </li> <li>Knowledge of laws, regulations, and accreditation standards related to medication management. </li><li>RP - Registered Pharmacist - State of Pennsylvania Upon Hire </li></ul><br><b>Preferred Qualifications</b><ul><li>5 years relevant pharmacist experience in patient care or population health or</li> <li>1 year Residency PGY-1 or</li> <li> formal training in population health. </li><li>Prior experience in working with health insurance payer contracts. </li> <li>Prior experience in Population Health. </li><li>BCPS - Board Certified Pharmacotherapy Specialist - Board of Pharmacy Specialties Upon Hire or</li> <li>BCACP - Board Certified Ambulatory Clinical Pharmacist - Board of Pharmacy Specialties Upon Hire </li></ul><br><b>Physical Demands</b><br>Lift and carry 25 lbs. frequent sitting/standing, frequent keyboard use, *patient care providers may be required to perform activities specific to their role including kneeling, bending, squatting and performing CPR.<br><br>Job Description Disclaimer: This position description provides the major <span style="overflow-wrap: break-word; display: inline; text-decoration: inherit; hyphens: auto;">duties/responsibilities,</span> requirements and working conditions for the position.
