Please highlight any secondary language fluencies on your application.</p><p style="min-height:1.5em"></p><p style="min-height:1.5em"><strong>About the Role</strong></p><p style="min-height:1.5em">1bios is a leading provider of Remote Patient Monitoring (RPM), Chronic Care Management (CCM), and other virtual care management technologies and solutions. Our Care Pros work under 1bios client physician and/or nurse general supervision and utilize the 1bios platform to deliver virtual care.</p><p style="min-height:1.5em">In this position, you will act as a virtual care coach helping people with chronic conditions. Care Coaches help onboard patients, deliver RPM and/or CCM programs, review and triage remote monitoring alerts, collaborate with an extended care team, coordinate care, follow and update patient care plans, and support patients in achieving their health goals.</p><p style="min-height:1.5em"></p><p style="min-height:1.5em"><strong>Important Note on Daily Volume:</strong> You should expect to spend most of your day on the phone and computer, including making approximately 30–70 outbound calls per day. This role is best suited for candidates who are highly comfortable in a fast-paced, high-volume, phone-based outreach environment.</p><p style="min-height:1.5em">We are looking for candidates who are available to begin within approximately 2 weeks of accepting an offer.
